The way I managed to quit smoking was Skinnerist: I pinched myself on the inner forearm whenever I had, or thought about having, a cigarette — and I consciously associated that pain with smoking and the idea/visualization of damage to my lungs (and the rest of my body).
Humans can learn very efficiently from pain. The hitch with smoking is that the feedback loop is far too slow.
