I currently keep a log of decisions in my notes app, with each individual "decision" note title starting with option-d on macOS, which gets you a ∂ character
Makes it very easy to scroll through and at least see the titles of decisions once a month during my review of notes.
Sometimes past decisions I was serious and sure about seem very uninformed or silly when I review the ∂ note later, but writing it down is the only way I can make the later comparison and realize it.
